Go see Myria the Shaman in Tri-Stone to obtain them.

Chaos Fangs/Level 1 Scythe - Complete the first game at least
once on any difficulty.
Pauldron of the Horseman/Level 5 Shoulder - Play the first game and earn at
least one achievement.
